Valid government ID with picture, Driver’s License, State I.D., or passport from your country. Vaccination record from your country or if you attended school in the U.S., please bring school vaccination record.

The physician will advise you which vaccines are needed based on the medical examination and age of the patient. The flu shot is required if the patient’s exam is between Oct. 1 and March 31st. Some medical insurance plans may cover vaccinations, but not the examination.

The I-693 form can be completed in approximately 4-5 working days from the initial exam date. The one thing that causes a delay in completing this form, is having to wait on the reports and/or results from the lab.

What is the purpose of Form N-648?
What is the purpose of Form N-648?
In general, applicants for naturalization and/or U.S. citizenship must demonstrate that they understand the English language, including the ability to read, write, and speak words in ordinary usage. They must also demonstrate knowledge and understanding of the fundamentals of the history, principles, and form of government of the United States. Together, these are the English and civics requirements for naturalization. This form is intended for applicants who seek an exception to the English and/or civics requirements due to a physical or developmental disability or mental impairment that has lasted or is expected to last, 12 months or more.

Proper government ID with picture, prior medical records that pertain to the disability, award letter from social security, etc. Our physicians may have to do an extensive medical records review prior to the appointment. The form may be completed within 2-3 days after medical examination of the patient.
